Qedani Dorothy Mahlangu served as Gauteng MEC for Health from 2009 to 2016. She was the primary decision-maker behind the termination of the Life Esidimeni contract in October 2015, which led to the transfer of approximately 1,700 mental health patients from licensed care facilities to unlicensed NGOs. At least 144 patients died. An inquest led by Judge Mmonye Teffo (concluded July 10, 2024) found Mahlangu and former Gauteng Health Department official Manamela negligent in the deaths of 9 patients. Mahlangu repeatedly refused to appear before the Teffo Inquest, citing her right to silence. The NPA was considering prosecution of 2 of the 144 deaths as of August 2025; no charges had been filed by April 2026.
