Albuminuria
Albuminuria — measured as urine albumin-to-creatinine ratio (UACR, mg/g) — is a biomarker of glomerular damage and an independent predictor of CKD progression and cardiovascular risk. In SURPASS-4 post-hoc analysis, tirzepatide reduced UACR by −6.8% vs a +36.9% increase with insulin glargine over 85 weeks. Pooled SURPASS-1-5 analysis found UACR reductions of −19.3% to −26.3% vs comparators. These are consistent and clinically meaningful signals, though the mechanism is partially mediated by weight loss and blood pressure reduction, rather than direct renoprotection.
