Dulaglutide
Dulaglutide (brand name Trulicity) is a once-weekly subcutaneous GLP-1 receptor agonist developed by Eli Lilly. In the SURPASS programme, it served as the active comparator in SURPASS-4 (vs tirzepatide in T2D with high cardiovascular risk) and as the active comparator in SURPASS-CVOT (the head-to-head CV outcomes trial, NEJM 2025). SURPASS-CVOT demonstrated that tirzepatide was non-inferior to dulaglutide on MACE-3 and numerically superior on the composite MACE-4 endpoint. Dulaglutide has established cardiovascular outcomes evidence (REWIND trial) and is on the ADA/EASD preferred list for T2D with high CV risk.
