• Is there a validated way to score erythema (or replace it) in EASI/SCORAD for toddlers with skin of colour? The “up-score by one level” advice (Sarkar 2026) is expert opinion, not a tested adjustment.
• Do the UK Working Party criteria, as adapted by NICE CG57 for under-4s, have lower sensitivity in Black and Asian toddlers whose eczema is extensor/follicular rather than flexural?
• How often is scabies, tinea or molluscum dermatitis misdiagnosed as eczema in 1–4-year-olds in primary care, and how often does that lead to steroid escalation?
