Insulin Degludec
Insulin degludec (brand name Tresiba, Novo Nordisk) is an ultra-long-acting once-daily basal insulin analogue. It was the comparator in SURPASS-3, which enrolled T2D patients on metformin with or without SGLT2 inhibitors who had not yet started injectable therapy. SURPASS-3 demonstrated tirzepatide (5, 10, 15 mg) was superior to titrated insulin degludec in HbA1c reduction and body weight at 52 weeks, with lower hypoglycaemia risk. The trial represented the first-injectable decision point in T2D management: whether to start a GLP-1/GIP agonist or a basal insulin.
