Hypoglycaemia Risk
Tirzepatide has a fundamentally lower risk of hypoglycaemia than insulin-based therapies because its insulin-releasing mechanism is glucose-dependent — it stimulates insulin secretion only when blood glucose is elevated. When glucose falls to near-normal levels, GIP and GLP-1 receptor stimulation of insulin secretion falls off. This is in contrast to exogenous insulin, which continues to drive glucose down regardless of prevailing glucose levels.
In SURPASS-1 (monotherapy vs placebo), no clinically significant hypoglycaemia was reported with tirzepatide. In SURPASS-2 vs semaglutide, hypoglycaemia rates were very low in both groups (0.2–1.7% vs 0.4%). The most clinically relevant comparison is SURPASS-6, where tirzepatide added to basal insulin produced 0.4 hypoglycaemia events/patient-year vs 4.4 events/year with prandial insulin lispro — an 11-fold reduction. This has major clinical implications for patients on insulin who fear hypoglycaemia or have had previous severe episodes.
