Sleep Disturbance in Eczema
Night-time itch from the Itch-Scratch Cycle wakes toddlers and their parents, and Eczema UK names sleep problems for “the person with eczema and their family” as one of the hardest parts of the disease. In the ALSPAC birth cohort (13,988 children followed from 2 to 16 years), children with active Atopic Dermatitis slept about as long as other children (−2 min/day) but had worse sleep quality: more night and early-morning waking, trouble falling asleep, and nightmares (aOR 1.48). Sleep was worse with severe eczema (aOR 1.68) or comorbid Asthma/Allergic Rhinitis (aOR 1.79), and was affected even in mild (OR 1.40) or inactive (OR 1.41) eczema (Ramirez, JAMA Pediatr 2019). Infants and toddlers. The toddler years are where the burden is clearest. In a 2026 controlled study of 55 infants aged 3–36 months with eczema and 60 healthy controls, children with eczema had shorter night sleep and longer night waking, and 56.4% were “poor sleepers” against 30.0% of controls. Sleep got worse as SCORAD severity rose (Tekcan, Pediatr Int 2026). This is a single-centre study in a setting where bed-sharing is common, which may magnify the effect on parents.

Parents and family. In the same study, 50.9% of parents of infants with eczema had poor sleep quality on the Pittsburgh index, against 28.3% of control parents, and a poor-sleeper child strongly predicted a poor-sleeping parent (Tekcan 2026). In US national survey data (NHIS 2013–2018), 45.2% of parents of a child with eczema slept under 7 hours a night, against 37.1% of other parents, and 13.3% against 9.9% had used sleep medication in the previous week (adjusted OR 1.26; Siegel, Pediatr Dermatol, via MedPage Today). “When a child has atopic dermatitis, his or her parents effectively have it as well” (Craiglow). That survey had no data on eczema severity, so it is cross-sectional evidence only.

Management — treat the skin first. The best evidence for better sleep is better eczema control. In an intensive treatment programme (29 parent–child pairs), clearing eczema improved the child’s sleep duration and efficiency on actigraphy, and improved child and parent sleep, daytime functioning and quality of life for up to 3 months (Meltzer, Ann Allergy Asthma Immunol 2020). That means proper Emollient Therapy and enough Topical Corticosteroids for flares, with Wet Wrap Therapy in severe flares. Eczema UK’s practical tips for night-time:
- keep nails short;
- use all-in-one sleep-suits with built-in mittens;
- use several light cotton layers rather than one heavy one (see Clothing and Fabrics);
- keep the bedroom cool and ventilated (Heat and Sweat worsen itch);
- try fridge-cooled emollient creams.

Sedating antihistamines — a short sleep aid, not an eczema treatment. NICE CG57 allows a 7–14-day trial of a sedating antihistamine during flares for children aged 6 months and over when sleep disruption is significant. It does not recommend routine use. The Cochrane review (Matterne 2019) found no convincing evidence that oral H1 antihistamines improve eczema itself. Sedating antihistamines can cause paradoxical excitation in young children, and some regulators bar them under age 2 (see Oral Antihistamines, including the guideline conflict). Any sleep medicine for a toddler needs a doctor’s advice.

What parents report (evidence: anecdotal). The parent of an 18-month-old on r/eczema gives “Benadryl most nights before bed or he’ll wake up screaming with blood all over the crib”. The parent of a 2-year-old found that applying cream and then a back massage helped him resettle after night waking. Other posters say antihistamines did not stop itch-driven sleeplessness.
