Clobetasone Butyrate

Clobetasone butyrate 0.05% (brand Eumovate) is a moderate-potency topical corticosteroid. DermNet places it at 2–25 times the strength of Hydrocortisone. It should not be confused with Clobetasol Propionate (Dermovate), which is very potent and differs by only a few letters. In NICE CG57’s stepped approach, moderate potency is used for moderate eczema in children. On the face it is used only for short 3–5-day courses in severe flares. In the axillae or groin it is used for 7–14 days. If a mild or moderate steroid has not controlled eczema within 7–14 days, the child should be reviewed.

OTC restrictions. In the UK, Eumovate Eczema & Dermatitis 0.05% Cream is sold in pharmacies without prescription. Its leaflet limits use to adults and children aged 12 and over: once or twice daily, for up to 7 days. It must not be used on the face, groin, genital area or between the toes, and should not be used under 12 “unless your doctor tells you to”. The prescription Eumovate cream lists eczema, dermatitis and Nappy Rash as uses. Its leaflet warns that a child’s nappy acts as an occlusive dressing, so more steroid passes through the skin. A toddler can therefore receive clobetasone only on prescription.

Duration in children. The prescription leaflet says treatment in children under 12 “should not normally exceed 7 days”. It adds that prolonged daily treatment “can affect their growth” and that continuous daily use beyond 4 weeks is not recommended. It prints the children’s Fingertip Unit table, measured on an adult finger.

Conflict: [Eumovate Rx PIL, emc 3807, n.d.] says moderate TCS courses in children under 12 should not normally exceed 7 days. [NICE CG57, 2007/2023] uses moderate potency for flares and reassesses only if not controlled within 7–14 days, continuing about 48 hours after symptoms subside. Unresolved — add to open_questions.

Hidden use. Clobetasone has been found undeclared in unlabelled “herbal” eczema creams (Ramsay ADC 2003). A parent using such a cream could be applying a moderate steroid with no limit on duration or site.

Clobetasone and other steroids have turned up in unlabelled “herbal” creams. Unsupervised daily use on a toddler’s face or under a nappy is a known route to Skin Atrophy and steroid side effects.

What parents report (evidence: anecdotal). UK parents often say hydrocortisone was too weak and that a step-up cleared the skin. One wrote: “We needed stronger creams than hydrocortisone. We were advised to really zap in then tail off gradually”. Another: “when doctor finally prescribed a stronger steroid cream it cleared up within a few weeks” (Mumsnet 2022).
