Borage Oil

What it is and the claimed mechanism. Borage (starflower, Borago officinalis) seed oil is the richest common source of Gamma-Linolenic Acid (GLA). It is sold as a capsule supplement for eczema alongside Evening Primrose Oil. The theory is that people with eczema convert linoleic acid to GLA poorly, so adding GLA should restore anti-inflammatory prostaglandin precursors such as DGLA (MSKCC). Herbalists have also used borage traditionally to induce sweating, as an expectorant and to promote lactation. Borage oil is sold as a food supplement, not a medicine, so it has no Traditional Herbal Registration and its contents are not checked by the MHRA. Evidence. The 2013 Cochrane review (Bamford et al.) pooled 8 borage oil RCTs and found no improvement in eczema over placebo. The largest included trial, Takwale 2003 (UK, BMJ), gave borage oil or placebo to 140 patients, 69 of them children, and found no difference. A 160-adult German trial (Henz 1999) also found no overall benefit, although a subgroup whose blood GLA metabolites rose did better. MSKCC’s herbal monograph agrees that the trials do not support its use for atopic dermatitis. This is a consistent null result from reasonably sized trials, not a lack of research. Toddler safety: the pyrrolizidine alkaloid question. Borage plants contain Pyrrolizidine Alkaloids (PAs) such as amabiline, lycopsamine and intermedine, which can damage the liver and possibly cause cancer. Risk rises with dose and duration. MSKCC states that 1–2 g of borage seed oil a day can give an unsaturated-PA intake approaching 10 µg, whereas the German health agency limit is 1 µg a day. It therefore advises using only products certified free of unsaturated PAs and avoiding the oil in pregnancy and breastfeeding. Because a toddler’s dose per kilogram of body weight is higher, and an oil with no proven benefit offers nothing to set against even a small liver risk, borage oil is a poor choice for a 1–4-year-old. Conflict: [MSKCC, n.d.] says borage seed oil contains small amounts of hepatotoxic unsaturated PAs (up to ~10 µg/day at 1–2 g oil) and recommends certified PA-free products. [ConsumerLab, n.d.] says PAs are not extracted with borage seed oil, so seed-oil products “would not be expected to contain PAs”. Unresolved — add to open_questions.
